MAINTAIN YOUR ELIGIBILITY FOR TIRES FOR LIFE


When you have purchased a new or used vehicle which means this dealership will replace any eligible tire(s) on your vehicle with tire(s) equivalent to those on the vehicle Purchase date if the tread depth of the tire(s) has worn evenly to 3/32" of an inch or less. Subject to availability and fulfillment of guidelines, the exact make/model tire or comparable tire will be installed. Maximum reimbursement will not exceed $150 per tire. Please allow up to six weeks for reimbursement. Tire(s) damaged due to Road Hazards are not eligible for repair and/or replacement under this Agreement (See Optional Protec Road Hazard) TO QUALIFY FOR REIMBURSEMENT ALL SCHEDULED MAINTENANCE SERVICES THAT ARE RECOMMENDED AND LISTED IN OUR SERVICE DEPARTMENT MAINTENANCE SCHEDULE BOOKLET MUST BE PERFORMED WITHIN SPECIFIC INTERVALS AT THIS DEALERSHIP IN ORDER TO MAINTAIN YOUR ELIGIBILITY FOR FREE TIRES FOR LIFE. ALL VEHICLES MUST BE DRIVEN A MINIMUM OF 18,000 MILES FROM DATE OF SALE TO BE ELIGIBLE FOR PROGRAM REDEMPTION. The undersigned understands that to qualify for the reimbursement of tire(s), you are required to follow these stated guidelines; 1). Have all regular scheduled maintenance as outlined in Our Service Department Maintenance Schedule Booklet performed at this Dealership. 2). Have the engine oil and filter (LOF) changed every 3 months or 3,750 miles at this Dealership. 3).  Have the tires rotated every other LOF or 7,500 miles at this Dealership. A grace period of 500 miles or 1 month is permitted. Failure to maintain proper alignment, causing irregular wear, will result in denial of tire(s) cost reimbursement. Our Service Department will validate the completion of each service visit with a validation stamp. This is a complimentary and optional program and has no cash value and is non-transferable. All claims are subject to the Administrator's prior approval. You will be responsible for the cost of mounting, balancing, and valve stem(s) of any tire(s) replaced by this Dealership under the Tires for Life Program.

OPTIONAL PROTEC ROAD HAZARD

 


New/Used Vehicle Tire and/or Wheel Guarantee: (If customer opts for optional road hazard) Flat tire repair or tire replacement when tire is damaged beyond repair as a direct result of a covered road hazard (debris on the road surface such as nails, glass, potholes, rocks, tree limbs or any other object or condition not normally found in the roadway). Tires must have at least 3/32 inch tread depth remaining. Wheel must be damaged so that it is unable to seal with the tire resulting in air loss.  Registered owner will be reimbursed the cost of a new tire and/or wheel, including mounting, balancing, valve stem and sales tax as shown on the sales invoice, or credit card payment will be issued for payment to the servicing dealership or repair facility.      
Notice: Wheels will be repaired whenever possible. Unrepairable wheels will be replaced with wheels of a like kind and quality, subject to availability of the vehicle's original wheels


EXEMPTIONS AND EXCLUSIONS: This does not apply to (a) Tire failure outside the Continental United States and Canada; (b) Destruction of the tire(s) due to impact with a curb or sidewall damage (does not qualify as a road hazard);(c) Replacement of tire(s) due to normal wear, by misalignment or suspension problems; (d) Rims leaks, manufactures defects, vandalism, fire, upset collision, theft or misuse by driving ob the tire(s) when flat; (e) Tires installed on trucks over 1 ton capacity and vehicles other than on-road passenger or recapped tire(s); (f) Failure occurring from operating on any surface other than Federal, State, County, City or Municipality paved roads or highways; (g) Tires transferred from vehicle on which they were originally installed or failure to file claim within 48 hours of occurrence; (h) This plan is excess to any other policy that may be in force during this plan. Should another policy be in force and claim occur, this policy will reimburse the individual named on this policy for the amount of their primary insurance deductible up to $250.00.


CLAIM PROCEDURE: Call administrator to obtain authorization before repair or replacement. If claim occurs outside of normal business hours, please call Administrator the next business day to report claim.
The following documents will be requested prior to authorization: (A) Copy of this contract; (B) Copy of sales invoice for replacement of tire and/or wheel with authorization number written on invoice; (C) Tread depth of damaged tire must be shown on sales invoice; (D) A completed and signed claim form. All paperwork must be submitted within 60 days of claim authorization. When a claim in filed, Administrator will verify the validity of the contract and contract owner information, then verify the failure with the Repair Facility, verify coverage and cost of repair/replacement, then authorize repair of covered parts. The Repair Facility will be provided with an authorization code and payment will be by credit card to the Repair Facility or by contract holder reimbursement.
CANCELLATION: This agreement is non cancel-able.
